At Cedar Hills Family Practice, we work hard to ensure that your time in the office is spent with your physician. We have provided many of the check-in forms online so that you can complete them prior to your appointment. By filling out these forms in advance, we are able to complete the check-in process in less time and allow you to see your healthcare provider sooner.

You can fax or email your forms to us when complete and we will register you in advance of your appointment. Please include a copy of your insurance card if possible.
